Predictably after 8 years it no longer works.
But OMG why isn’t this a core feature of Jetpack forms already? This is a totally useful thing, thanks for trying.
Gives this error message:
Sorry something went wrong,
either you don’t have any results
or you don’t have Jetpack installed.
Double check you have both of these and try again.
